Call for Proposals - Dementia Programming

October 4, 2018 - November 7, 2018

In support of the North Simcoe Muskoka Local Health Integration Network’s (NSM LHIN) 2016-19 Integrated Health Service Plan (IHSP) and Ontario’s Dementia Capacity Planning Project (DCP), the NSM LHIN is issuing a Call for Proposals (CFP) to identify opportunities to increase system capacity for community dementia programs in the NSM LHIN. 

Overview

Community dementia programs provide support for people living with dementia and their care partners, including caregiver education and support and those referred to as “adult day programs” and/or “dementia day programs”.  This CFP is seeking proposals that will increase access to community dementia programs including day, evening and/or overnight programs and services.  Proposals may also support increased access to transportation services for clients who experience barriers in getting to or from community dementia programs.

Eligibility Criteria

The applying organization must:  

  • Be a health service provider with an existing Multi-Service Sector Agreement (M-SAA);
  • Allocation of existing staff resources is not eligible for funding;
  • Be able to implement the proposed program in fiscal 2018-2019; and,
  • Demonstrate past compliance with LHIN reporting requirements and achievement of program deliverables.
