Call for Proposals - Lead Agency - Specialized Geriatric Services
June 30, 2015 - August 6, 2015
The North Simcoe Muskoka Local Health Integration Network (NSM LHIN) is requesting proposals from LHIN-funded health service providers interested in being the Lead Agency for the planning and implementation of the new integrated regional Specialized Geriatric Services Program in NSM, the first building block of a broader integrated regional seniors health program.
To be eligible, it is mandatory that organizations be LHIN designated and funded health service providers. The organizations must also hold a current Service Accountability Agreement (SAA) with a LHIN. These two requirements must be met for inclusion in the evaluation process.
In addition to the mandatory eligibility criteria listed above, organizations must have proven success in planning and implementing regional programs. The organization must be able to demonstrate leadership and commitment to the care of frail seniors and their caregivers.
