Call for Proposals - Lead Agency - Rapid Access Clinic: Low Back Pain

In support of the North Simcoe Muskoka Local Health Integration Network’s (NSM LHIN) 2016-19 Integrated Health Service Plan (IHSP) and in response to an identified service need, the NSM LHIN is issuing a Call for Proposal (CFP) directed to Primary Care organizations.

Overview

In mid-2017, the Ministry of Health introduced a provincial initiative to implement an assessment and intake program across the province called Rapid Access Clinics (RACs). Through 2018-20 the RAC model of care, will be implemented across the province in the areas of low back pain and hip and knee. This call for proposal is specific to low back pain.

 

To stabilize and advance the planning and implementation of a Musculoskeletal (MSK) standardization in North Simcoe Muskoka, the NSM LHIN is seeking a single health service provider to become the Lead Agency accountable for the MSK Rapid Access Clinic for Low Back pain program in the NSM LHIN.

Eligibility Criteria

  • The submitting organization must be a community based health care organization;
  • The proposal must be submitted under an existing NSM LHIN Service Accountability Agreement in good standing or in partnership with an NSM LHIN health service provider with a Service Accountability Agreement in good standing;
  • Must be able to implement the proposed program by March 31, 2019;
  • Compliance with reporting requirements and achievement of program deliverables; and
  • Have existing resources and demonstrated ability to manage new IT platforms.
